Solicited Proposals

Applicants who have been asked by CHCF to submit a proposal and detailed budget will find important information and downloads to guide them through that process. CHCF requires the following documents with grant proposals.

1. Project cover sheet. Download and complete the Project Cover Sheet form to provide us with summary information about your project. This form can be used for Letters of Intent and Grant Proposals. Save the form with your organization's name in the filename before emailing it to us.

2. Tax ID information.

  • Nonprofits should submit a copy of the IRS determination letter.
  • For-profit entities or consultants should submit a completed W9 form.

3. Proposal guidelines. Download and follow the Proposal Guidelines for staff-solicited projects. We appreciate that not all items listed will be relevant to all proposals submitted to CHCF.

4. Proposal budget form. Use the Proposal Budget form to submit your budget request. For multi-year projects break out each year of the project on separate budget forms.

Note: This form is not required for consultants who are paid an hourly rate, but a detailed budget should still be submitted with your proposal.

Allowable CHCF indirect cost rate: As indicated on the budget request form, CHCF allows up to 15% of direct costs (excluding subcontracts) to be charged to a project as an indirect cost.

Questions about the project should be addressed to CHCF staff who requested the proposal. For general questions, such as how to complete the forms, contact grants@chcf.org. Proposals may be submitted to the appropriate staff.
